ESM,MUMBAI MOVES TO THE GRAND NEW OFFICE : INAUGURATED

In a special gesture of goodwill
and support, Chief executive of Maritime and Port Authority of
Singapore RAdm(NS) Lui Tuck Yew flew
in to the commercial capital of Mumbai to inaugurate the new premise
of Executive Ship
Management’s office at Govandi on 05th October,2004.
To reciprocate this grand
gesture, RAdm(NS) Lui and his team comprising Mr. Lee Seng Kong,
Senior Director, MPA , Mr. Gerald Loh ,Manager, MPA and Mr. Niti Nandan,
Consul General of S'pore in Mumbai ,were given a rousing reception by
the staff of ESM, Mumbai as well as the Faculty and staff of Samundra
Institute of Maritime Studies (SIMS).
Speaking to the distinguished
guests many of whom had come in from various parts of India and abroad
to participate in the event, RAdm Lui was generous in his praise for
Executive Ship Management and its leadership for the quality and
standard of its operation. He congratulated the company for all along
sustaining a planned progress together with focus on high quality.
RAdm Lui also pointed out to the recent ASL award given
to ESM for its contribution to the economy of Singapore.
Needless to say, the function was
held at the brand new auditorium with 100 seating capacity on the
first floor of the new building.
RAdm Lui and his team also
visited the adjacent trainings institute SIMS and were visibly
impressed at the facilities, which were considered some of the best in
the international maritime training field.

FLOODING
OF ENGINE ROOM DURING MAINTENANCE:
Narrative
:
Routine maintenance of the
engine room main sea strainer was in progress. During the work the
crewmember working on the strainer was called away for another job,
leaving the strainer open. Apparently unaware of the maintenance work
going on, one of the engineers opened the sea valve next to the
strainer from the control room, resulting in ingress of water through
the open strainer. The high-level bilge alarm was automatically
activated but was acknowledged by one of the engineers without further
action.
Extent Of Damage :
The
engine room was flooded to the top of the main engine. Although
immersed for a short time only, all motors, generators, starters,
cables and other electric equipment had to be overhauled and renewed
in parts or total.

LESSONS LEARNT:
This is one of
several incidents over the years where opening up of sea water systems
during maintenance has caused flooding of the engine room as a result
of operational mistakes, seriously endangering the safety.
The underlines the
importance of:
1.
Adequate procedures for safeguarding of maintenance work by
instituting adequate control and co-ordination during such jobs on
board.
2.
The safety management system on board which must ensure that
these procedures are properly implemented.
3.
Particular focus and attention on maintenance of systems, which
may give the sea direct access, like the main sea water system.
