ESM: PROUD RECIPIENT OF FIRST MANNING LICENSE
ISSUED BY GOVT. OF INDIA
Executive Ship Management became the first ever company to have received the Manning License issued by the government of India under the Merchant Shipping Rule, 2005 for recruitment and placement of Indian seafarers.

Currently, ESM is one of the largest recruiter of Indian seafarers having full Indian crew on board all ESM managed foreign going vessels including those of oil major BP and other international clients.

An audit was concluded by a Senior Principal Surveyor of IRS (Indian Registry of Shipping) on 26th August, 2005 with nil non-conformity and a confirmation of compliance was sent to the DGS on the spot.

Mr. I.N. Bose, Senior Principal Surveyor, IRS carried out the audit and followed the relevant MS Rules 2005 in detail. Several random checks of procedures and documents were made including computer entries.

Along with the manning division, our HSEQA dept. extended full assistance and cooperation to make the audit a hundred percent success.

FOLLOWING CORRECT PROCEDURES

The oil fired boiler had failed and was urgently needed as the ship was required to manoeuver. The junior engineer was trying to investigate this fault. While trying to find the fault, the junior engineer tried to ignite the boiler. The burner unit was not secured to the boiler and the combustion chamber had not been purged.

Unfortunately, the junior engineer succeeded in igniting the boiler and the residual fuel ignited explosively. The force of the explosion detached the burner unit striking the junior engineer in the chest and face. The flame from the explosion burnt his left hand, wrist and arm and the force of the impact knocked him over backwards, which caused a head wound. He was taken to the ship's hospital for treatment.

Had the burner been properly secured - the explosion would have been contained.

The junior probably thought that he was being quick, efficient and helpful. Instead he very nearly delayed the ship, leaving her short handed.

LESSONS LEARNT
All of the failures were human. No amount of perceived pressure should be allowed to influence safe working practices which must be followed at all times.
Senior engineers must be informed immediately in case of boiler failures and manual firing of the boilers must be done only in their presence.
Ensure that the burner is correctly secured before putting boiler into use.
Ensure that adequate purging is carried out prior to ignition.
TAKE 5 must be carried out before starting on a job.
